2           有効性や安全性、品質に関する情報を共有するためにも、互いの医療機関で開催される際には、少なくとも互いの倫理審査委員会で行われた議論の内容がわかるような書面を提示するなどして、相手の医療機関における実施体制等について理解することが必要である。その上で、相手側の倫理審査委員会の要請がある場合等には、医療機関の関係者が出席し、各医療機関における実施体制等について説明可能な体制を互いに協力して整備すること。
2           It is necessary to inform and understand the implementation systems of the medical institutions to share information on efficacy, safety, and quality, by means including but not limited to presenting the content discussed in the ethics committee meeting when they are held. On top of that, the medical institution related personnel should be present, and cooperate to establish a system on understanding the implementation system in medical institutions when requested by another ethics committee.

3           一般的には、第１の２に示すように、調製実施医療機関において第１の各号に示す倫理審査を経て、既に一定の有効性及び安全性が確認されたものが、他の治療実施医療機関でも提供されると考えられる。そのような、調製実施医療機関の倫理審査委員会による審査がより専門的で適切であることも考えられる場合にあっては、治療実施医療機関から調製実施医療機関の倫理審査委員会へ審査を委託することも考慮すべきである。
3           Generally, as showed in item 2 of paragraph 1, products that have been confirmed to have efficacy and safety with the ethical review specified in paragraph 1, can be provided to other medical institutions performing therapy. Medical institution performing therapy should consider the entrustment for medical institution performing preparation when more professional and appropriate review than that of the ethics committee in medical institution performing preparation is needed.

4           複数の医療機関において共同で実施する場合においても、調製の段階が分断されるのではなく、原料となる患者自己免疫細胞・組織の採取から、調製、搬送、投与までに至る各過程が一貫して複数の医療機関により実質的に管理されていることが必要である。共同での医療の実施は、複数の医療機関の関係者が１つのチームとなり、当該関係者がすべての患者の症例を把握しているなど十分な連携体制（顔の見える関係）の中で実施されることが必要である。
4           Even in the situation where preparation is performed by multiple medical institutions, the stages of preparation should not be separated, but need to be under practically management by these medical institutions through every process from harvesting of personalized immune cell and tissue, to preparation, transportation, and administration. It is necessary for the preparation to be implemented in multiple medical institutions where related personnel should unite as a team and comprehend all patient cases, making a coordination system (having face-to-face relationship).

5           医療関係者の連携については、複数の医療機関において共同で一体となって患者自己免疫細胞療法を実施する場合には、特に重要であり、患者の診療情報を両医療機関の関係者が共有した上で、患者の治療や治療後のモニタリングを共同で実施し、各々の医療機関での記録の保存を考慮すること。
5           It is vitally important to implement personalized immune cell therapy by multiple medical institutions jointly, so that treatment and monitoring after the treatment can be jointly performed even though the medical information of the patient has been shared by the related personnel, so that records can be saved by each medical institution.

例えば、主治医を中心として両医療機関の医師又は歯科医師の参加によるカンファレンスを実施した上で治療方針や重大な事態が生じた場合の対応の決定等を行うことが必要である。
For example, it is necessary to decide the treatment strategy and response of serious incidents through conference by physicians or dentists in both medical institutions which are led by attending physician.

6           両医療機関の関係者は、当該免疫細胞療法に適切と考えられる期間、共同で有効性や安全性に関して患者をフォローすること。
6           The follow-up of the patients receiving immune cell therapy should be jointly performed by related personnel in both medical institutions for an appropriate period of time.

7           両医療機関の医師又は歯科医師は、実施する患者自己免疫細胞療法に関する知識・技能（細胞・組織の調製に関する事項を含む。）を有すること。
7           Physicians or dentists in both medical institutions should have the knowledge and skill on implementing personalized immune cell therapy which include items on preparation of cells and tissues.

8           細胞調製管理、品質管理、及び必要に応じてバリデーション等に関することについても、予め両医療機関で共有することが必要である。
8           It is necessary to share information in advance between two medical institutions, including information on cell preparation management, quality management, and validation if required.

9           調製実施医療機関が調製した細胞・組織を他の治療実施医療機関に提供する場合には、第１の２に示すように、一定の有効性及び安全性が確認されたものが提供されるべきである。したがって、調製実施医療機関についても、少なくとも十分な有効性及び安全性が確立されていない段階（臨床研究や評価療養）においては、細胞・組織の調製のみに特化することなく、自ら実際にこれを用いた医療を実施し、十分な評価を行っていること。
9           The cells and tissues prepared by medical institution performing preparation can be provided to medical institution performing therapy if their efficacy and safety have been confirmed with the item 2 of paragraph 1 above. Thus, medical institution performing preparation should not only focus on specializing preparation of cells and tissues, but also focus on the practical implementation in medical care with such product to get sufficient assessment in stages where efficacy and safety have not been completely established which may include clinical researches and treatments to be evaluated.

10       実施した患者自己免疫細胞療法に関する成績の評価やホームページでの公表については、複数医療機関で連携して実施することも検討する必要がある。
10       It is necessary to review both the implementation of the therapy through the coordination of multiple medical institutions, and the assessment of the performance of implemented personalized immune cell therapy and its announcement on the homepage.
