全ての調製プロセスは、責任医師の監督の下で行われること。調製実施医療機関は、本ガイドラインに従い、調製プロセスの特徴に応じた一貫性のある品質システムを構築すること。

第１ 取り違え・混同防止対策
細胞・組織の採取や調製プロセスにおける取り違え、及び混同を防止することは、患者自己免疫細胞療法の安全性確保の上で最も重要な管理項目の一つである。

1           患者識別情報
調製プロセスにある細胞・組織に係る患者の識別については、患者を判別でき、かつ、取り違え及び混同を確実に防止するために適切な情報により行うこと。

Identification of patients pertaining to cells and tissues in the preparation process should be performed based on the appropriate information where patients can be distinguished and prevention of mix-ups and confusion can be reliably prevented.

＜具体例＞
医療機関での調製・投与にあたり、調製プロセスにおける患者識別情報として患者名を使用する場合には、同姓同名者との取り違えを防止するため、ID 番号を併記するなどの配慮が必要である。ただし、二重盲検試験などにおいては、医療機関内であっても患者名は使用できず、匿名化された識別番号などを使用すべき場合も考えられる。個人情報が特定できない記号や番号等を用いた患者識別情報を用いる場合には、異なる患者から採取した細胞・組織と取り違えを起こす可能性のある紛らわしい記号、番号等の使用は避けること。いずれの場合も、ラベルは使用環境と期間による劣化が認められない材質を使用し、貼り替えや改ざん等が行われたことが容易に認識できるラベル等を用いることが望ましい。

2           作業区域等
細胞・組織と、当該細胞・組織に係る患者識別情報とが、常に適正な対応関係で移動することを担保し、取り違えを確実に防止するために、以下に掲げる事項に留意し、必要な措置を採ること。
2 Working area and others
The following items should be considered and necessary measures should be taken to ensure that cells and tissues and patient identification information pertaining to the cells and tissues are always moved in an appropriate correspondence to prevent mix-ups.

（1）          無菌操作等の、容器の開放を伴う操作を行う重要区域においては、同時に複数の異なる患者から採取した細胞を取り扱うことができないように、厳密に管理すること。

（2）          気密又は密封容器が使用されている場合は、重要区域以外のインキュベーター等の培養装置内において、複数の異なる患者から採取した細胞を同時に取り扱うことを否定するものではない。但し、取り違え及び混同防止のための手順を予め規定し、確実にすること。
(2) When tight or hermetic containers are used, it is not prohibited to handle cells collected from multiple patients at the same time in incubators or other culture devices outside of critical areas. However, procedures to prevent mix-ups and confusion should be specified and determined in advance.
