Patient: Hello, my name is Hanako Tanaka, date of birth is June 8th, 1989.
Doctor: How would you like me to address you?
Patient: You can call me Hanako.
Doctor: Alright Hanako, how can we help you today?
Patient: I’ve been having heavy menses.
Doctor: Oh no, for how long already?
Patient: For the last 3 months or so.
Doctor: You mentioned heavy menses. How many days do you bleed?
Patient: The first 5 days.
Doctor: Is it heavy throughout the 5 days? Or just the first few days?
Patient: Just the first 2 days or so.
Doctor: Alright, during the heavier days, how many times do you have to change pads or tampons?
Patient: I have to change around 6 times or so.
Doctor: Are the pads fully soaked?

Doctor: That’s great to hear. Finally, what do you work as, Hanako?
Patient: I’m an actress.
Doctor: Alright Hanako, based on what you’ve told me, there could be several reasons for your heavy menses. This could be a structural cause where there could be fibroids in the womb or polyps in the cervical area. Or this could be a systemic cause due to imbalances in your thyroid hormone levels. Either way, we need to perform some blood tests and examine your pelvic area. Is that OK with you?

プレゼン例１

32 year old healthy Japanese primigravida presented with heavy painless menses. Her last menstrual cycle was 2 weeks ago, lasting 5 days, consisting of 2 days of heavy bleeding and 3 days of moderate bleeding. Previously the patient changed the pads 2-3 times even on the heavier days. However, for the past 3 months, heavier menses requires her to change pads 6 times per day, with pads fully soaked, mixed with occasional clots. She has mild dizziness on the first few days of her menses but denies dyspnea. Otherwise, she has no intermenstrual bleeding or vaginal discharge. The patient underwent menarche at age 13, which since has been regular and occurs every 4 weeks. She is sexually active with 2 male partners without experiencing any episodes of dyspareunia or post-coital bleeding. Her last Pap-smear taken last year was normal. She does not have any medical and surgical history, but has a positive family history of cervical cancer in her mother, who had hysterectomy in her late 50’s. 
On review of symptoms, she denies loss of consciousness, palpitation, headache, cough, nausea/vomiting, dysuria, hematuria, and dyschezia.

32 year old previously healthy female, P0, presents with first onset painless menorrhagia for 3 months duration. She reports bleeding for the first 5 days with heavy flow for the first 2 days. During the first 2 days she requires 6 pads each day, fully soaked with clots. Prior to this, she only required 2-3 pads per day. This is not associated with any abdominal pain, vaginal discharge, dyspareunia or dyschezia. The patient reports some anemic symptoms including giddiness but no chest pain, SOB, palpitations and LOC. ROS is unremarkable and the patient does not report any recent weight loss.
The patient does not have any past medical or surgical history. Regarding her gynecological history, menarche was at the age of 13 and her periods are usually regular, coming about every 4 weeks. She is currently sexually active with 2 partners and does not report any post-coital bleeding or intermenstrual bleeding. Pap smear done last year was normal. Patient has a positive history of endometrial cancer in her mother who underwent hysterectomy in her late 50s.
